Initiatives
The alliance started with a workshop in December 2007. This workshop highlighted patient needs through real patient stories demonstrating the day to day struggles of living with diabetes. It also highlighted the difficulties faced by health care practitioners in preventing and treating diabetes. Over the course of two days, eleven innovative solutions were identified, modeled, and prioritized. During 2008, Healthy Hoosiers focused on three projects to meet the needs of Hoosiers with diabetes or at risk for developing diabetes. The projects are as follows:
Nutrition in the Fast Lane - Available now
Rather than saying "Don't eat fast food" Healthy Hoosiers has focused on helping people choose wisely while they are in a fast food restaurant.
The goals of the guide are:
- Encourage healthy eating by making wise choices at fast food restaurants
- Teach healthier eating behaviors at younger ages so healthy habits are formed early
- Introduce the Healthy Hoosiers collaboration. To date the guides have been distributed at
several major events such as:
- Diabetes camps and events
- Marion County Health Department
- Cultural Expos
- State and local fairs
Anthem IVR Program - IVR calls will begin in 2009
- Anthem as part of its involvement with the Healthy Hoosiers Alliance will be providing Interactive Voice Response calls to remind policyholders of common lab tests that should be obtained when you have diabetes
Healthy Hoosiers Diabetes Toolkit - under development
- A diabetes resource toolkit, including web based materials, for health care practitioners and patients
- Educational and clinical resources as well as standardized outcome measures and reporting tools
- Focused on managing diabetes across the continuum of care
